Step 1: Applicant Information
An example of an applicant information field.
State of Residence: Your state of residence is required to receive a term life insurance quote. Some life insurance companies do not offer certain products in all states.
Gender: Life insurance companies use different premium rates for males and females. Additionally, there are different height / weight requirements based on gender.
Date of Birth: : Your age is an important factor in determining a term life insurance premium. Some companies quote by a process called Age Nearest Birthday, which makes it very important to enter the exact date of birth.
Height and Weight: Your height and weight are used to determine your rating class. Height/weight requirements vary by the life insurance company, so you may get a Preferred rate quote from one company and a Standard rate quote from another.
Tobacco Use: This is another important factor in determining your rating class.
Please select the correct time frame for your last tobacco/nicotine use (this includes nicotine patches, gum, etc.). Tobacco/Nicotine use guidelines vary by the life insurance company, so you may get a Non-Tobacco rate quote from one company and a Tobacco rate quote from another.
Step 2: Quote Specific Information

Rating Class [ ? ]: Your rating class determines the premium rate for your term life insurance policy. Preferred Plus has the lowest premium rates while Standard premium rates are higher. Sub-standard premium rates are higher than Standard, but they are not quoted on the website. To assist you in determining your rating class, we have created a rating class chart that lists various guidelines to follow.

Coverage Amount [ ? ]: This is the amount of life insurance coverage you are seeking. The more coverage requested, the higher the premium rates will be. To help determine the amount of coverage needed, you can use our Term Life Insurance Estimator.
Length of Coverage [ ? ]: This is the amount of time the term life insurance policy will be in force. This is displayed in years, such as 10, 20, etc. The longer the length of coverage, the higher the premium rates will be.
Premium Payment Mode [ ? ]: This determines how often you will make life insurance premium payments to the insurance company. Available options are annual, semi-annual, quarterly and monthly, with annual the least expensive option as shown here.
