All insurance quotes and products are offered through QuickQuote Financial, Inc., licensed agency and Tim Bain, licensed agent in all 50 U.S. states and the District of Columbia.

Premium rates are current and accurate, as provided by QuickQuote Financial, Inc.'s partner insurance companies. The premium rates shown are for quoting purposes only, are not an offer of coverage and are not guaranteed. Final premium rates will be determined by the insurance company and are based on individual circumstances. As a result of underwriting, final premium rates may differ from quoted premium rates.

QuickQuote is committed to the protection of your personal information. This privacy statement describes how we collect, use, protect, and store the information you provide to us. It also explains the steps you can take to review, modify, and delete your information. This statement is available on our homepage and at all points on our Web site where personally identifiable information is requested.

By participating in the Council of Better Business Bureau's BBBOnLine ® Privacy Program we have made a commitment to meet the programs' strict requirements regarding how we treat your information and have it verified by BBBOnLine. Children's Privacy

Protecting the privacy of children is especially important. For that reason, we never collect information on our website from those we actually know are under 13, and no part of our Web site is structured to attract anyone under 13.

Information Collection

QuickQuote is the sole owner of the information collected on www.quickquote.com. QuickQuote collects information from our users at several different points on our Web site. We only collect information that is necessary for us to provide the user with the most accurate quotes possible and to fulfill our responsibilities as the user's representative throughout the application process.

Quote Questionnaire

We request information from the user on our Quote Questionnaire Form. A user must provide basic demographic information (such as state of residence, county of residence, and zip code), personal information (such as date of birth, gender, and tobacco/nicotine use), and private health information (such as current medical conditions). This information is used to provide quotes for the selected insurance products and is necessary to provide the most accurate quote possible. At no point on the questionnaire is the user required to provide personally identifiable information (such as name, social security number, address, telephone number, and email address).

Application Request

We request information from the user on our Application Request Form. A user must provide contact information (such as name, telephone number, mailing address, and email address) and unique identifiers (such as a password). This information is used to complete the user's request for insurance applications and establish an account for the user to track the status of the applications. The information is used to contact the user if there are problems processing requests, to follow up on applications sent, to follow up on applications received, to service the user during the application (underwriting) process, and to service the user beyond the closing of the insurance transaction.

Online Applications

Our Web site offers optional online applications (Virtual Forms) for many of the insurance products we market. We request information from the user on our Virtual Forms. A user must provide contact information (such as name, telephone number, mailing address, and email address), personal information (such as date of birth, place of birth, gender, tobacco/nicotine use, citizenship status, driving record, etc.), personally identifiable information (such as social security number and driver's license number), private health information (such as current/past medical conditions, current/past medications, and current/past physicians), and financial information (such as credit card number number/expiration date). This information is used to populate the insurance company's paper applications which are then either printed by the user or mailed to the user.

Information Use

All information collected is used to provide the user with quotes on insurance products, to process insurance application requests, to assist with application completion, to assist insurance companies with application processing, and to provide customer service support. We do not use information for any other reasons.

We do not sell, distribute, or otherwise make available user information to outside parties except to the extent necessary to perform the above listed duties.

Communications

Our users will only receive communications directly from us, from the insurance company they have applied to, or from our service providers.

    Users that Request Quotes Only

Users that request quotes only will not receive any follow up communication from us in any form. We do not collect personally identifiable information or contact information for quote requests.

    Users that Request Applications

Users that request applications will receive communications from us by email, U.S. mail, and telephone. An initial email will be sent to confirm receipt and fulfillment of the user's request. Follow up emails will be sent weekly for a period of 60 days until the application is received back in our office. A telephone call will be made to the user after 60 days confirming their desire to either submit or cancel their application.

    Users that Submit Applications

Users that submit applications will receive communications from us by email, U.S. mail, and telephone. Such communications will be made on an as needed basis to assist with processing the application and facilitate the underwriting process. Examples of such communications are to seek additional/missing information, update the user as to the status of the application and to provide the user with the insurance company's decision and/or offer.

    Customer Service

We communicate with users on a regular basis to provide requested services and in response to user requests. We will reply to requests by email or telephone, in accordance with the user's wishes.

Choice / Opt-out

Users who wish to opt-out of certain types of communications may do so by sending us an email. Users must specify which type of communication they wish to opt-out of (such as telephone or email). Users must allow at least one form of communication in order for us to fulfill our responsibilities.

Corrections/Updates

Users are able to access, correct, and update personally identifiable information collected by us. Access is granted online via our Customer Recognition System, which is password protected. Once inside the system, users can click on the link to change/modify information. Changes can also be made by sending us an email or by telephone at 800-867-2404.

Deleting / Deactivating

Users may request to delete and/or deactivate personal information from the site's database by sending us an email or by telephone at 800-867-2404.

Cookies

A cookie is a piece of data stored on the user's computer tied to information about the user. Usage of a cookie is in no way linked to any personally identifiable information while on our site. We use persistent cookies for those users that choose to establish an account for recording quotes. A persistent cookie is a small text file stored on the user's hard drive for an extended period of time. Persistent cookies can be removed by following Internet browser help file directions.

If a user rejects the cookie, they may still use our site. None of our business partners use cookies on our site.

Log Files

Like most standard Web site servers, we use log files. This includes internet protocol (IP) addresses, browser type, referring pages, platform type, date/time stamp, and number of clicks to analyze trends, administer the site, track user movement in the aggregate, and gather broad demographic information for aggregate use. IP addresses, etc. are not linked to personally identifiable information. We use a tracking utility called Webalizer that uses log files to analyze user movement. We do not track the movement of individual users.

Profile

A profile is stored information that details a user's viewing preferences. We do not profile individual users on our Web site, with the exception of users that choose to establish an account for recording quotes. Once a user establishes such an account, we will link collected information to that user's personally identifiable information to provide a record of quotes for the user.

Sharing

    Legal Disclaimer

Though we make every effort to ensure user privacy, we may need to disclose personal information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our Web site.

    Aggregate Information (non-personally identifiable)

We share aggregated demographic information with our partner insurance companies and advertisers. Such information is used to determine the accuracy of quoting, appropriateness of products offered, effectiveness of marketing programs, and effectiveness of our Web site. This is not linked to any personally identifiable information.

Instances in Which We Share Information

    Partner Insurance Companies

We share personal information with partner insurance companies for users that submit applications for insurance. The information shared is the information provided by the user on the insurance application and supplementary forms. In all cases, the user has the opportunity to review the application and supplementary forms for completeness and accuracy. All forms are required to be physically or digitally signed by the applicant prior to submission to the insurance company. The shared information is used to process the user's request for an insurance product.

This transfer of information is a requirement for the insurance application process. If a user does not agree to this transfer, we will not be able to process the user's application reques.

Partner insurance companies will use and retain the user's information as per their own privacy policies. QuickQuote does not control the privacy practices of our partner insurance companies.

   Partner Web Sites

QuickQuote may occasionally work with partner Web sites to provide some insurance products. If a user requests an insurance quote supported by a partner Web site, the user will be directed to the partner's Powered By system for fulfillment. Information will be collected by the partner to process the user's request.

Any information collected is subsequently owned by the partner Web site and falls under the guidance of the partner Web site's Privacy Policy. QuickQuote does not control the privacy practices of our partner Web sites.

    Service Providers

We partner with service providers to provide paramed exam services for certain types of insurance products. When a paramed exam is required by a partner insurance company, we provide the service provider with the user's contact information (such as name, address, and telephone number) that is necessary for them to provide their services.

Links

This Web site contains links to other sites. Please be aware we are not responsible for the privacy practices of such other sites. We encourage our users to be aware when they leave our site and to read the privacy statements of each and every Web site that collects personally identifiable information. This privacy statement applies solely to information collected by this Web site.

Surveys

Our site occasionally requests information from users via surveys. Participation in these surveys is completely voluntary and the user therefore has a choice whether or not to disclose this information. The requested information typically includes feedback on the user's experience while on our site. Survey information will be used for purposes of monitoring or improving the use and satisfaction of this site. Users' personally identifiable information is not shared with third parties unless we give prior notice and choice. Though we may use an intermediary to conduct these surveys, they may not use users' personally identifiable information for any secondary purposes.

Security

QuickQuote takes every precaution to protect our users' information. When users submit sensitive information via our Web site, their information is protected both online and off-line.

Online Encryption

When our application forms ask users to enter sensitive information (such as social security number or medical information), the information is encrypted with and protected by the best encryption software in the industry - SSL (Secure Sockets Layer). The lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked while on secure pages, such as our application forms. Our Web site displays the VeriSign Secure Site Seal to show that our site is authentic and all transactions are secured by SSL. Offline Protection

We incorporate the highest security measures reasonably possible within our offices to protect user information. Only employees who require information to perform a specific job (such as Licensed Insurance Professionals and Customer Service Representatives) are allowed access to personally identifiable information. Employees must provide a unique login and password to access our customer database. Employees are directed to logout of the database when away from their desks and must login again when they return. All employees are kept current on our security and privacy practices.

Paper applications, forms, and other physical materials containing personally identifiable information are kept in locked file cabinets when not in use. All premium checks received from users are kept locked in the company safe until they are forwarded to the respective insurance company (usually within 24 hours of receipt in our office).

All incoming callers are asked to verify their identity prior to receiving information from our Licensed Insurance Professionals or Customer Service Representatives.

Our office building is locked from the outside after normal business hours. Our office suites are also locked after normal business hours and protected by a monitored intrusion alarm system.

All documents containing personally identifiable information that are not required to be kept in a customer's file are immediately shredded in-house.

Systems Protection

Electronic data is stored on our secure servers, which are double-locked in a server cage at our collocation facility. Access to the servers is password protected. The servers are equipped with firewall protection and the latest stable versions of server software.

HIPAA Compliance

The Department of Health and Human Services (HHS) published a final regulation called the Privacy Rule in December of 2000, in response to the Health Insurance Portability and Accountability Act (HIPAA) of 1996. The Privacy Rule became effective on April 14, 2003. The rule set national standards for the protection of consumers' Private Health Information (PHI) on the part of health plans and other health care providers that conduct business electronically.

QuickQuote has implemented the required standards and is compliant with all aspects of the Privacy Rule.

Notification of Changes

Any changes to our privacy policy will be posted to this privacy statement on our homepage and other places we deem appropriate. Our users will always be aware of what information we collect, how we use it, and under what circumstances we disclose it. We will use information in accordance with the privacy policy under which the information was collected.

If we intend to use users' personally identifiable information in a manner different from that stated at the time of collection, we will notify users via email. Users will have a choice as to whether or not we use their information in this different manner. If users have deleted/deactivated their account, then they will not be contacted, nor will their personal information be used in this new manner. If we make any material changes in our privacy practices that do not affect user information already stored in our database, we will post a prominent notice on our Web site notifying users of the change.
